This form is for medical professionals to refer clients in need of home safety modification services.
If you are a medical provider or caregiver working with a senior whose home environment could benefit from safety modifications — like grab bars, ramps, improved lighting, or other accessibility upgrades — you can refer them to us using this form. Once submitted, a TruBlue home safety specialist will reach out promptly to schedule a free, no-obligation assessment of the home.
During the assessment, we’ll evaluate potential fall hazards and accessibility barriers, then provide a customized recommendation — whether that’s grab bars, ramps, stair modifications, bathroom safety upgrades, or other improvements to make daily living safer and easier.
After the assessment, we’ll walk the homeowner (or caregiver) through the recommended modifications and explain how our maintenance plans can help keep their home safe, functional, and comfortable in the long term. Our goal is to promote safe aging in place — because a home-made safe today can help prevent falls and injuries tomorrow.
